Consent
Wireless DNA, a distributor of Rogers, is collecting and processing your personal information. By completing this form, you will be subscribing to postpaid mobile service with Rogers, operated by Rogers Communications (βRogersβ).
The following information is being collected in order to set up your account:
- Name, your address while in Canada, email, date of birth, your expected arrival and departure dates, passport number and study permit number.
This information is being collected for the following purposes:
- To assist in setting up your Rogers account
- To establish an eligibility relationship with you
- To conduct a credit check
- To provide service
Who is your information being shared with?
- Wireless DNAβRogers's dealer, who will be processing your wireless application.
- External credit bureaus to determine your eligibility for service.
- Fido and its parent company, Rogers to deliver services to you.
In order to complete the set-up of your account, your personal data will be transferred to Canada in accordance with our privacy policy. Your personal data will be retained by Rogers Communications located at 333 Bloor St E, Toronto, ON M4W 1G9.

Your rights
You can exercise the following rights with respect to the personal data provided to us. You may do so by writing to the Grievance Officer whose details are provided above.

Right to nominate: You have the right to nominate any individual who will exercise your rights set out under this consent form in case of your death or incapacity (i.e., inability to exercise statutory rights granted to a data principal due to unsoundness of mind or infirmity of body).

Right to access information about your personal data: You have the right to obtain a summary of such personal data and the processing we have undertaken in relation to the same. You also have the right to obtain from us (a) the identities of the parties with whom we have shared such personal data along with the description of the personal data shared; and (b) any other information as may be required to be provided to you under applicable law, unless we have shared such personal data with another person who is authorised by law to obtain such personal data for the purpose of prevention or detection or investigation of offences or cyber incidents, or for prosecution or punishment of offences, as applicable.

Right to correct and erase personal data: You have the right to correct, complete, update and/or require erasure of your personal data. However, we will not be able to comply with an erasure request if we have to retain such information for the purpose for which it was collected or for complying with legal requirements.

Right to withdraw consent: You agree that your provision of this consent is voluntary, and you also have the right to withdraw your consent to the processing of your personal data. You may do so by writing to the Grievance Officer using the contact details described above. Please note, however, that this may prevent us from effectively offering our services or fulfilling our obligations pursuant to our contractual relationship with you and/or complying with law, which may mean we are unable to continue offering our services or contractual relationship.
